Indiana is in the Great Lakes region of the United States, with shoreline on Lake Michigan. The state is located in the Corn Belt, and most years has almost half of its cropland devoted to corn. Residents of Indiana are known as Hoosiers, which is also the team name for Indiana University athletic teams. Indiana covers 35,867 square miles, with a 2010 estimated population of 6,483,802 people – 1,405,057 living in rural Indiana (USDA-ERS). Indianapolis, the capital, is located in the central part of the state. The state’s largest cities are Indianapolis, Fort Wayne, and Evansville. According to the U.S. Census Bureau, 84.3% of the state’s population is white, 9.1% is African-American/Black, 1.6% is Asian, 0.3% is American Indian, and 6.0% is of Hispanic/Latino origin (2010).
There are 123 hospitals in Indiana, 38 of which are located in rural areas (Indiana State Office of Rural Health, 2012). The state has 35 hospitals currently identified by the Flex Monitoring Team as Critical Access Hospitals (March, 2011). There are 64 Rural Health Clinics in Indiana (Kaiser, 2012), and 19 Federally Qualified Health Centers provide services at 89 sites in the state (Kaiser, 2011). Most Hoosiers have some form of health insurance coverage, although 14.0% of the state's residents lack any health insurance (Kaiser, 2009-2010).
According to the USDA Economic Research Service, the average per-capita income for all Hoosiers in 2009 was $34,022, although rural per-capita income lagged at $30,129. 2010 estimates indicate a poverty rate of 14.7% exists in rural Indiana, compared to a 15.4% level in urban areas of the State. 2010 ACS data reports that 14.0% of the rural population has not completed high school, compared to 13.8% of urban populations. The unemployment rate in rural Indiana is at 11.0% while in urban Indiana it is at 9.9% (USDA-ERS, 2010).

Lifestyles Diabetes Project
