Purpose

Vitalyst Health Foundation provides Systems Change Grants for collaborative work designed to transform systems through changes to policies and practices to improve health in Arizona communities. Applicants from tribes and rural areas are encouraged to apply.

Successful applications will propose projects designed to make a significant, sustainable impact that address the intersection of 2 or more elements of a healthy community through an equity lens.

Systems Change projects address the following areas:

  • Increase capacity to identify and address issues affecting community health and well-being
  • Strategically leverage infrastructure, policies, and systems design to improve health where people live, learn, work, pray and play
  • Improve environmental, social, behavioral, and health infrastructure conditions that are determinants of health and have significant impact on Arizonans' well-being and resiliency
  • Create sustainable solutions to problems that endure after the term of the grant funding
Eligibility

Nonprofit organizations working in Arizona are eligible to apply.

Application process

Follow the steps for the discovery process linked from the program website. Staff will be in contact to schedule a 30 minute conversation via phone call or Zoom.

Submit a letter of intent through the online portal by October 28, 2022. Selected applicants will be invited to submit a full proposal with a deadline of January 27, 2023.
