Purpose

The Rural Health Care Coordination Program provides seed funding for rural health consortiums to improve and expand delivery of health care services through comprehensive care coordination strategies in rural areas.

Program goals include:

  • Expand access to and quality of equitable health care services through care coordination strategies exclusively in rural areas
  • Utilize an innovative evidence-based, promising practice, and/or value-based care model(s) that is known to, or demonstrates strong evidence to, improve patient health outcomes and the planning and delivery of patient-centered health care services
  • Increase collaboration among multi-sector and multidisciplinary network partnerships to address the underlying factors related to social determinants of health
  • Develop and implement deliberate and sustainable strategies of care coordination into policies, procedures, staffing, services, and communication systems

Year one of the grant is intended for planning activities, with years 2-4 focused on implementation. Applicants must select one of the following as their primary focus area:

  • Heart disease
  • Cancer
  • Chronic lower respiratory disease
  • Stroke
  • Maternal health

Eligibility

Eligible applicants are domestic public or private, non-profit or for-profit entities, including faith-based, community-based, tribes, and tribal organizations.

Additional requirements include:

  • Located in an eligible rural area
  • In a consortium/network with at least 3 or more healthcare providers
  • Have demonstrated experience serving, or the capacity to serve, rural underserved populations
  • Have not previously received an award for the same or similar project unless the applicant is proposing to expand the scope of the project or the area that will be served through the project
