Purpose

The Rural Health Care Services Outreach Grant Program is a community-based program designed to expand, enhance, and sustain healthcare delivery in rural communities.

The overarching goals for the program are to:

  1. Expand the delivery of healthcare services to include new and enhanced services exclusively in rural communities
  2. Deliver healthcare services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and delivery of services
  3. Utilize and/or adapt an evidence-based or promising practice model(s) in the delivery of healthcare services
  4. Improve population health, and demonstrate health outcomes and sustainability

In addition to funding programs through the Regular Outreach track, applicants may take part in a national effort that targets rural health disparities through a second track called the Healthy Rural Hometown Initiative (HRHI). HRHI seeks to address the underlying factors that are driving growing rural health disparities related to the five leading causes of avoidable death, which include heart disease, cancer, unintentional injury/substance use, chronic lower respiratory disease, and stroke.

Eligibility

Eligible applicants must:

  • Be a domestic public or private, nonprofit or for-profit entity with demonstrated experience serving, or the capacity to serve, rural underserved populations
  • Represent a network composed of participants that: (1)Include at least 3 or more healthcare provider organizations; and (2)Are rural, urban, nonprofit, or for-profit entities, with at least 66% of network members located in a HRSA-designated rural area
  • Not previously received a grant under this program for the same or similar project, unless the entity is proposing to expand the scope of the project or the area that will be served through the project

Healthy Rural Hometown Initiatives (HRHI)Track:

  • Award ceiling: $250,000 per year
