| Purpose |
Nonprofit or public hospitals with 100 or more beds that do not offer a formal palliative care program are invited to apply for a Sojourns Pathway implementation grant. Preference will be given to hospitals that receive a Sojourns Pathway planning grant and develop a plan to launch a palliative care program. Grantees receive funding to:
- Launch a palliative care program in the hospital
- Provide palliative care services to patients, and collect data to support the program’s ongoing operations
- Explore hospital-community palliative care partnerships

| Eligibility |
Nonprofit or public hospitals with 100 or more beds that do not offer a formal palliative care program, or whose PC program has been active or intermittent for two years or less.
Any hospital meeting the eligibility requirements listed
below may apply for an implementation grant. However, hospitals that first receive a Sojourns
Pathway planning grant and complete the required two-day Palliative Care Leadership Center
training will receive special consideration from the Foundation.
To be eligible to apply for an implementation grant, hospitals must:
Be a nonprofit or public hospital with 100 or more beds
Serve communities in Washington, Oregon, Idaho or Utah
