Purpose he goal of The Commonwealth Fund's Program on Health Care Disparities is to improve the quality of health care delivered to low-income and minority Americans, and to eliminate racial and ethnic health disparities. The program builds on efforts to improve quality of care overall in the United States, by focusing on hospitals and ambulatory care providers serving large numbers of low-income and minority patients (minority-serving, safety-net institutions). The strategies it pursues include:
  • Identifying opportunities for improving performance of safety-net providers;
  • Enhancing the capacity of safety-net providers to improve performance; and
  • Fostering incentives and policies that promote better performance of safety-net providers.

    The Program on Health Care Disparities is interested in funding the following types of projects:

  • 1. Assessments of the current level of performance among minority serving safety-net providers, and factors associated with high performance in quality, patient experience, and efficiency.
  • 2. Evaluations of innovative models and practices that lead to high performance among minority-serving safety-net hospitals and ambulatory care providers.
  • 3. Assessments of the impact of current payment policies, particularly through Medicaid and Medicare, on safety-net provider performance.
  • 4. Evaluations of health reform and its impact on safety-net provider performance.
  • Eligibility The Fund awards grants to tax-exempt organizations and institutions and public agencies. If an organization does not have 501(c)3 status or other tax-exempt recognition, the Fund will determine whether a grant is possible based on a review of the project and the organization's expertise in conducting the research.

    Occasionally the Fund enters into contracts with for-profit organizations when a suitable nonprofit research base is not available.

    During fiscal year 2004–05, the median award amount for board-level grants, which are awards in amounts greater than $50,000, was $210,965. For small grants, in amounts less than or equal to $50,000, the median award was $18,324. Grant periods range from one month to a few years, depending on the scope of the project. The Fund awards grants that support a complete project period, and not annual, renewable grants.

    process
    The Fund strongly prefers grant applicants to submit letters of inquiry using the online application form. The form is available at: http://www.commonwealthfund.org/loi/

    While the Fund will continue to accept letters of inquiry via regular mail and fax, such submissions will take longer to process than those received online. Letters of inquiry are acknowledged when received. Applicants are typically advised of the results of an initial staff review within two months. Program staff will contact applicants if more detailed information is required. Proposals recommended by Fund staff are reviewed and voted upon by the Board of Directors, which meets three times each year. However, letters of inquiry are accepted on a rolling basis; there are no deadlines.
