Purpose The Office of Rural Health Policy's Rural Health Care Services Outreach Grant Program encourages the development of new and innovative health care delivery systems in rural communities that lack essential health care services. The emphasis of this grant program is on service delivery through collaboration, requiring the lead applicant organization to form a consortium with at least two additional partners. The community being served must be involved in the development and ongoing operations of the program, to appropriately address the needs of the population. Programs funded have varied greatly and have brought care that would not otherwise have been available to at least two million rural citizens across the country. Through consortia of local providers and others, rural communities have managed to provide services such as hospice, dental care for children, and prenatal care in many remote areas.

ORHP does not anticipate holding a competitive funding cycle for the Outreach program in FY 2010 and FY 2011. However, grantees awarded in FY 2009 will be eligible to apply for the non-competitive continuation cycle in FY 2010 and FY 2011. It is anticipated that the next competitive cycle for this program will be in FY 2012.

Eligibility Eligible lead applicants are rural, nonprofit private or public entities that represent a consortium of three or more organizations that deliver health care services in rural areas. Only the applicant organization is required to be a rural private or public not-for-profit entity. Faith-based and community-based organizations are eligible to apply.

To be eligible, the grant recipient's organizational headquarters must be a rural public or rural nonprofit private entity and must be located in a rural county or in a rural census tract of an urban county. All of the proposed services must be provided in a rural county or census tract. Exceptions to this are 330(g) Migrant Health Clinics that exclusively provide services to migrant and seasonal farmworkers in rural areas or a Federally recognized Native American Tribal Government providing all of the proposed services on Federally-recognized Tribal land.

In addition to the 50 States, applicants can be located in the District of Columbia, the Commonwealth of Puerto Rico, the Commonwealth of the Northern Mariana Islands, the Territories of the Virgin Islands, Guam, American Samoa, the Compact Free Association Jurisdictions of the Republic of the Marshall Islands, the Republic of Palau, and the Federated States of Micronesia, if they meet eligibility requirements.

The legislation for this program states that applications should be prepared in consultation with the appropriate State Office of Rural Health (SORH) or other appropriate State entity. Each State has a SORH and the ORHP recommends contacting the SORH or other State entity early in the application process to advise them of your intent to apply. The SORH may be able to provide some technical assistance to applicants.
