Alabama Department of Public Health: Office of Primary Care and Rural Health
Facilitates and supports activities which improve access to health care services for rural Alabamians, with special concern concern for children, the elderly, minorities and other medically underserved vulnerable populations.
Alaska Office of Rural Health
Collects and disseminates rural health care information and data; is engaged in health care provider recruitment and retention; provides technical assistance to rural providers, hospital and communities; and coordinates rural health policy analysis.
Arizona Center for Rural Health
Promotes the health of rural and medically underserved individuals, families, and communities through service, education, and research.
Arkansas Office of Rural Health & Primary Care
Promotes the development of community-based health care services and systems throughout Arkansas to ensure that well managed, quality health services are available to all citizens. This is a component of the Arkansas Department of Health and Human Services, Division of Health.
California State Office of Rural Health (CalSORH)

Colorado Rural Health Center (CRHC)
An independent, non-profit, membership-based organization that serves as the State Office of Rural Health for Colorado. Offers programs and services to ensure that rural communities have access to adequate healthcare.
Connecticut State Office of Rural Health
Is located at Northwestern Connecticut Community College in rural Winsted, CT. The Office works closely with the Department of Public Health, Connecticut Primary Care Association, AHED, Department of Mental Health and Addiction Services, CT Hospital Association and community organizations promoting quality healthcare for rural residents.
Delaware Office of Primary Care & Rural Health

Idaho Bureau of Rural Health and Primary Care

Illinois Center for Rural Health
Works to improve access to primary health care in rural and underserved areas of Illinois. The Center for Rural Health is also the Primary Care Office for Illinois and is responsible for designation of provider shortage areas. A division of the Department of Public Health.
Indiana State Office of Rural Health (InSORH)
Promotes the health of rural residents through public health services, funding sources, education, conferences, publications, and various initiatives. Oversees the Indiana Rural Hospital Flex Program and the Small Rural Hospital Improvement Grant Program. Located within the Indiana State Department of Health's Primary Care Office.
Iowa State Office of Rural Health (IA SORH)
Helps rural communities and organizations identify and resolve issues, and build rural health infrastructure. Provides rural health advocacy and outreach, coordination of rural health resources, and consultation to communities and health care providers in rural Iowa communities. Part of the Bureau of Oral and Health Delivery Systems, which in turn is part of the Iowa Department of Public Health.
Kansas State Office of Rural Health (KS SORH)
Serves rural communities in Kansas. Collects and disseminates health information and links rural communities to programs that support rural health. Works with Kansas rural health clinics, CAHs, FLEX, rural hospitals, and underserved areas. Part of the Kansas Bureau of Community Health Systems, which is under the Kansas Department of Health and Environment.
Kentucky State Office of Rural Health
Partners with hospitals, community organizations, and non-profit groups, in promoting health care in rural Kentucky. Part of the Center of Excellence in Rural Health run by the University of Kentucky Medical Center.
Louisiana Bureau of Primary Care and Rural Health

Maine Office of Rural Health and Primary Care

Maryland Office of Rural Health

North Dakota State Office of Rural Health

Ohio State Office of Rural Health (OHSORH)

Oklahoma Office of Rural Health (OORH)
Coordinates, plans, and promotes quality healthcare for targeted, underserved rural Oklahomans. Works with communities to offer programs that train physicians and other healthcare providers, increase access to healthcare, and educate the public about rural health.
Oregon Office of Rural Health
Coordinates efforts to improve rural health systems, provides technical assistance to rural communities, recruits health care providers, advises hospitals about the Critical Access Hospital option, and serves as a clearinghouse for information and data on rural health issues.
