As the Sunshine State, Florida is noted for its warm climate, beautiful beaches and numerous recreational opportunities. Florida covers 53,927 square miles, with a 2010 estimated population of 18,801,310 people, including 1,207,042 people living in 33 rural Florida counties (FEDR, 2010). An additional 2.1 million persons were estimated by the U.S. Census to live in the rural portions of Florida’s 34 urban counties. Tallahassee is the state capital and the largest cities are Jacksonville, Miami, Orlando and Tampa. According to the latest estimates, 75.0% of the state’s population is white, 16.0% is Black/African American, and 22.5% is of Hispanic/Latino origin (FEDR, 2010).
There are 187 hospitals in Florida, 21 of which are located in rural areas (SORH, 2012). The state has 13 hospitals identified by the Flex Monitoring Team as Critical Access Hospitals (Sept. 2010). There are 143 Rural Health Clinics in Florida (Kaiser, 2011), and 44 Federally Qualified Health Centers provide services at 290 sites in the state (Kaiser, 2010). All of Florida’s 67 counties are designated in part or whole as primary care health professional shortage areas (HPSAs). Florida has the third highest percentage (25%) of uninsured, non-elderly persons in the country as of 2009 (Kaiser).
According to the USDA Economic Research Service (ERS), the average per-capita income for Florida residents in 2009 was $38,965, although rural per-capita income lagged at $29,537. Estimates from 2010 indicate a poverty rate of 20.2% exists in rural Florida, compared to 16.3% in urban areas of the state. 2010 ACS data reports that 19.8% of the rural population has not completed high school, compared to 14.3% of urban populations. The unemployment rate in rural Florida is 11.0%, while in urban Florida it is 11.6% (USDA-ERS, 2010).

The Rural Health Network of Monroe County sought to develop and sustain a countywide infrastructure for providing primary health care services to the county’s uninsured inhabitants.
Farmworker Case Management Across Borders
