Hawaii, the Aloha state, comprises of eight islands in the Pacific Ocean. Hawaii’s landscape is unique in that it exhibits towering volcanoes, lush valleys, and sandy beaches. It is the only state that has its own official native language. Hawaii covers 6,423 square miles, with a 2008 estimated population of 1,288,198 people – 383,164 living in rural Hawaii (USDA-ERS). Honolulu, the capital, is located on the island of Oahu. The state’s largest cities are Honolulu, Hilo, Kailua, Kaneohe, and Waipahu. Hawaii is the most racially and ethnically diverse state in the nation. According to the U.S. Census Bureau, 29.1% of the state’s population is white, 39.9% is Asian, 8.9% is Native Hawaiian or other Pacific Islander, 2.9% is African American/Black, and 8.2% is of Hispanic/Latino origin (2007).
There are 23 hospitals in Hawaii, 13 of which are located in rural areas (North Carolina Rural Health Research and Policy Analysis Center, Dec. 2008). The state has 9 hospitals currently identified by the Flex Monitoring Team as Critical Access Hospitals (April, 2009). There are 2 Rural Health Clinics in Hawaii (CMS, 2009), and 14 Federally Qualified Health Centers provide services at 82 sites in the state (Kaiser, 2008). Most Hawaiians have some form of health insurance coverage, although 8.3% of its residents lack any health insurance (Kaiser, 2006-2007).
According to the Economic Research Service, The average per-capita income in 2007 for all residents of Hawaii was $39,242 although rural per-capita income lagged at $32,614. 2008 estimates indicate a poverty rate of 11.1% exists in rural Hawaii, compared to a 8.5% level in urban areas of the state. Data from 2000 finds that 16.1% of the rural population has not completed high school, while 15.2% of the urban population lacks a high school diploma. The unemployment rate in rural Hawaii is 4.9%, while in urban Hawaii it is 3.5% (USDA-ERS, 2008).

Success Stories
Adolescent Behavioral Health Project
This project was designed to provide adolescents
with an employment experience at the health center to help build
self-esteem, develop healthy decisionmaking capabilities, and
prevent the onset of chronic health problems. The medical center
provided a range of behavioral health services to students at the two
local schools, those participating in the employment program, and
young children at home.
Angel Flight West: A Higher Level of Kindness
