Arizona Diamondbacks Foundation Program Grants

Healthy Start Eliminating Disparities in Perinatal Health (Border and Alaska)
The purpose of this program is to address significant disparities in perinatal health including disparities experienced by Hispanics, American Indians, African-Americans, Asian/Pacific Islanders, and immigrant populations.

Johnson & Johnson Community Health Care Program
Funding to assist community-based, non-profit organizations whose primary goal is to provide creative and effective access to quality health care for the medically underserved.

Primary Care Mini-Grant
Funding for local primary care needs assessments, community primary care strategic planning, community primary care improvement, community primary care identification of alternatives and resources & community program planning, implementation & evaluation.

Rural People, Rural Policy Participation Request Statement Grant
A multi-year national initiative that energizes and equips rural organizations and networks to shape policy that will improve the lives of rural people and the vitality of rural communities.

State and Regional Primary Care Association (PCA) Cooperative Agreement (Limited Competition)
Supports the capacity of state and regional primary care associations to provide technical assistance to health centers.

UnitedHealth HEROES Service Learning Grants
Funding to support youth-led projects that address childhood obesity.
