Iowa is known as the Hawkeye State, as a tribute to Indian leader Chief Black Hawk. Iowa is bordered by the Mississippi and Missouri Rivers. This state ranks first in the nation with corn and soybean production as well as in hog production from its 93,000 farms. Iowa covers 55,869 square miles, with a 2011 estimated population of 3,062,309 people - 1,321,044 living in rural Iowa (USDA-ERS). Des Moines, the state capital, is located in the south-central region of the state. The state's largest cities are Des Moines, Davenport and Cedar Rapids. According to the U.S. Census Bureau, 93.0% of the state’s population is white, 3.1% is Black/African-American, 0.5% is American Indian, 1.9% is Asian, and 5.2% is of Hispanic/Latino origin (2011).
There are 118 hospitals in Iowa (Kaiser, 2010), and two VA hospitals. The state has 82 hospitals identified by the Flex Monitoring Team as Critical Access Hospitals (March 2013). 56 rural hospitals are also recognized as the public health agency for the county. There are 142 Rural Health Clinics in Iowa (Kaiser, 2012), 18 regional and affiliate AgriSafe Clinics serving farm and agricultural clients, and 13 Federally Qualified Health Centers provide services at 83 sites in the state (Kaiser, 2010). Most Iowans have some form of health insurance coverage, although 11.0% of its residents lack health insurance (Kaiser, 2011).
According to the USDA Economic Research Service, the average per-capita income for Iowans in 2010 was $38,039, although rural per-capita lagged at $35,795. Estimates from 2010 indicate a poverty rate of 12.5% exists in rural Iowa, as well as 12.5% level in urban areas of the state. 2010 ACS data reports that 11.4% of the rural population has not completed high school, compared to 8.9% of urban populations. The unemployment rate in rural Iowa is at 6.1% while in urban Iowa it is at 5.7% (USDA-ERS, 2011).

Using the power of group purchasing within a cooperative structure, the North Central EMS Cooperative (NCEMSC) is an Emergency Medical Services (EMS) cooperative developed to serve Minnesota and surrounding states.
Maximizing Independence of Rural Elderly Through Use of Assistive Technology and Access to Health Care Services
Five organizations joined together to address the health care needs of the aging rural Iowa population by providing assistive technologies and home modifications, increasing access to primary health care and rehabilitative therapies, and educating rural elderly and health care workers to help identify potential hazards in homes.
Wayne County Multi-Generational Behavioral Health Project
This project aimed to increase access to behavioral health care for children, youth, and isolated elderly members by providing outreach and education resources, and promoting greater community involvement in an integrated network of services.
