    Based on the high incidence of diabetes in these counties, a consortium of agencies was created increase the knowledge of local physicians regarding appropriate management of diabetes, increase patient awareness of the need for treatment and how to manage diabetes properly,and to increase public awareness of diabetes, the risk factors for the disease, and where to go for testing.

  • Smiles on Wheels Mobile Dental Program

    The purpose of this project was to implement a mobile dental service, Smiles on Wheels, that emphasized community-based oral health education targeting low-income preschool children through third-graders as well as pregnant women living in poverty in five rural counties in the Florida Panhandle.
  • Southwest Georgia Multicultural Health Initiative

    The Southwest Georgia Multicultural Health Initiative was created to ensure that translation and interpretation assistance was available to individuals with limited English proficiency seeking care at local hospitals and health departments, to improve providers’ understanding of cultural diversity issues related to the people they serve, and to ensure that people with limited English proficiency have access to routine health education and screening services.
  • Washington County Rural Health Outreach Grant Program

    The goal of the project was to enable residents to access health care and education services—either by transporting residents to locations where these services were offered or by providing these services closer to where the county’s most rural residents live via clinics and outreach events.
