The goals of the Allendale Women’s Health Project were to increase the availability and accessibility of primary health
care services to women, improve birth outcomes and reduce infant mortality in
Allendale County, and to improve the over all health status of women living in the
county.
- Charlie's Place: Targeting Adolescent and Community Health

Charlie’s Place was established to promote healthier lifestyles
among the county’s adolescents.
- Samaritan North Lincoln Hospital Community Clinic

The Samaritan North Lincoln Hospital Community Clinic was established to provide primary medical and preventive services for
low-income and uninsured residents.
- The Diabetes Project

Based on the high incidence of diabetes in these counties, a consortium of agencies was created increase the knowledge of local physicians regarding
appropriate management of diabetes, increase patient awareness of the need for treatment and
how to manage diabetes properly,and to increase public awareness of diabetes, the risk factors for
the disease, and where to go for testing.
- Healthy Schools - Healthy Kids

The project was designed to reduce alcohol, tobacco, and
drug use; to address unsafe sexual behaviors; and to improve
healthy diets and physical activity among children and adolescents.
- Meadows Regional Medical Center Rural Health Outreach Project

The cosortium provided a wide range of primary
health care services, including general health, blood pressure, and
blood glucose screenings to
uninsured and underinsured families living in the service area.
- Rural Health Outreach Program for Southeastern North Carolina

The goals of the project were to expand access to care, restrain
costs, and improve the quality of services delivered to Hispanic
individuals and families living in the service area.
- Skipa

The Southern Kentucky Initiative for Pediatric Asthma
(SKIPA) project conducted home visits to provide caregivers with
knowledge, training, and support to decrease emergency
department visits, hospitalizations, and school absenteeism related
to asthma.
- Smiles on Wheels Mobile Dental Program

The purpose of this project was to implement a mobile dental
service, Smiles on Wheels, that emphasized community-based oral
health education targeting low-income preschool children through
third-graders as well as pregnant women living in poverty in five
rural counties in the Florida Panhandle.
- Southwest Georgia Multicultural Health Initiative

The Southwest Georgia Multicultural Health Initiative was
created to ensure that translation and interpretation assistance was
available to individuals with limited English proficiency
seeking care at local hospitals and health departments, to improve providers’ understanding of cultural diversity
issues related to the people they serve, and to ensure that people with limited English proficiency have
access to routine health education and screening services.
- Washington County Rural Health Outreach Grant Program

The goal of the project was to enable residents to access health
care and education services—either by transporting residents to
locations where these services were offered or by providing these
services closer to where the county’s most rural residents live via
clinics and outreach events.
- West Middle School Wolverine Den of Health

Local Organizations joined forces to establish a school-based health center at West
Middle School in Montgomery County that focused on the
health care needs of 6th, 7th, and 8th graders.
