- Neighborhood Meth Watch Community Policing Project

Adelante Juntos Coalition (AJC) designed and implemented the Neighborhood Meth Watch Program in many of the rural communities of Pinal County, Arizona.

The Initiative increased
awareness and access to health promotion services by providing ongoing leadership training regarding
healthy lifestyles for local youth; encouraging these youth to take a leadership role in planning,
implementing and monitoring local health promotion/education projects; and supporting these youth as
they plan and coordinate an ongoing local healthy lifestyles education outreach campaign for the youth in
our service area.

The main goal of this project was to help create a community-based
health care system that was better equipped to meet the health care
needs of a more racially, ethnically, culturally, and linguistically
diverse population in rural Virginia.

to help medical providers educate patients and to conduct
community outreach about diabetes.

rural areas in the region. The project has emerged as a model of community cooperation
and persistence in addressing unmet health care needs in a large,
rural, underserved area.

Toombs Teen Health Collaborative in an attempt to reduce teen pregnancy, reduce repeat births by teens,and to implement a system to improve
community-based health care services to teenagers.

Midwest Medical Center was the first Critical Access Replacement Hospital to open its doors in the State of Illinois. They overcame a tremendous amount of challenges to improve and save health care in this small rural and tourist community.
- Affordable Workforce Housing in Blackduck, Minnesota

An urgent need for affordable workforce housing in Blackduck, Minnesota was solved by an ambitious four-year project to build a 21-unit subdivision by 2005.
- Dickenson County Virginia Child Care Center
June 2007
Several organizations in Dickenson County Virgina worked together to build a state-of-the-art child care facility to serve workers at technology-based customer-service call centers.
- Better Health for the Delta
December 2006
The Southeast Louisiana AHEC grantee for the Louisiana Mississippi Delta initiative combined our activities during Phase I and Phase II with the Robert Wood Johnson RWJ) Southern Rural Access Grant Program (SRAG) rural loan fund component and created "Better Health For the Delta".
