- Inter-Lakes Dental Clinic

The clinic offered the full range of dental services, including
fillings, sealants, root canals, extractions, crowns, bridges, dentures,
cleanings, and checkups. Clients served by the clinic were of all
ages and income levels, although the project placed strong
emphasis on serving indigent patients covered by Medicaid or
Child/Family Health Plus.

Using morbidity, cost, and prevalence data, stakeholders selected four chronic conditions for quality improvement: pediatric asthma, diabetes, coronary artery disease, and depression.
