The Community Clinic Grant Program provides funding to plan, establish, or operate clinical services for low-income populations and/or populations living in rural areas of Minnesota.
Eligible projects include:
- Mental health screening and follow-up services for adolescents and young adults ages 12-25
- Oral health programs, with preference for projects supporting collaborative practice dental hygienists to provide services in community locations
- Clinical services for at-risk youth
Eligible applicants include:
- Nonprofit clinics established to provide preventative, medical, dental, or mental health services to low-income or rural population groups
- Government entities operating a clinic which provides preventative, medical, dental, or mental health services
- Indian Health Services units or tribal governments operating a clinic which provides preventative, medical, dental, or mental health services
- Consortia of any of the above entities
Award ceiling: $45,000
Expected number of awards: 10-13
Total funds available: $530,000
