Purpose

The Healthy Start: Eliminating Disparities in Perinatal Health program provides grants to improve health outcomes before, during, and after pregnancy and reduce racial/ethnic disparities in rates of infant death and adverse perinatal outcomes. Funding is set aside for rural communities.

Focus areas:

  • Providing direct and enabling services:
    • Increase receipt of case management/care coordination to facilitate access to medical care and community-based resources
    • Increase uptake of healthy behaviors before, during, and after pregnancy
    • Increase the use of healthy and safe infant and toddler care practices
  • Convening community consortia:
    • Advise and inform strategies for providing direct and enabling services
    • Develop cross-sector partnerships to ensure access for participants to coordinated, comprehensive maternal, child, and family medical care; health and parenting education; and community-based resources within the project area
    • Participate in Communities of Practice with other projects to develop and implement a plan for the community that focuses on at least one social determinant of health (SDOH), such as access to adequate housing, transportation, or food
Eligibility

Eligible applicants include any domestic public or private entity, including faith-based and community-based organizations, tribes, and tribal organizations.

Support beyond the first budget year will depend on:

  • Appropriation
