Purpose

The Rural Hospital Planning and Transition Grant Program supports small hospitals in developing strategic plans for improving access to health services, or in implementing transition projects to modify the type and extent of services provided, based on an existing plan.

When developing or implementing a plan, applicants must address:
  • Improving community access to hospital or health services
  • Changes in service populations
  • Availability and upgrading of ambulatory and emergency services
  • Extent that the health needs of the community are not currently being met by other providers in the service area
  • Need to recruit and retain health professionals
  • Extent of community support
  • Integration of healthcare services and coordination with local community organizations
  • The financial condition of the hospital
Eligibility

Eligible applicants are small, rural hospitals that fulfill all of the following criteria:

  • Non-federal, nonprofit, general acute care hospitals
  • Located in a rural area or located in a community with population of less than 15,000 outside of the 7 county Twin Cities metropolitan area
  • Have 50 or fewer beds

Estimated award range: $25,000 - $45,000
Project period: 1 year
Estimated number of awards: 10
Estimated total program funding: $250,000

At least one-half of the total cost of the project must be matched from non-state sources.
