The North Carolina Community Health Grant Program provides funding for nonprofit primary care safety net organizations to increase access to primary medical and preventive care.
Primary care safety-net organizations are eligible to apply for funding to pay for patient care through:
- Track A: Payment per face-to-face patient encounter (including telehealth) for low-income, uninsured, and underinsured residents who do not have healthcare coverage or access to primary healthcare services
- Track B: A reimbursement model for eligible expenses that may include payment for clinical staff, medical/office supplies and equipment, telehealth services, and capital
Nonprofit primary care safety net organizations that provide direct patient care and serve as a medical home are eligible to apply. This includes:
- AHEC clinics
- Federally Qualified Health Centers and look-alikes (FQHCs)
- Free and charitable clinics
- Health departments
- Nonprofit hospital-owned primary care clinics
- Rural health centers
- School-based and school-linked health centers
- Other nonprofit community organizations that serve uninsured and medically indigent persons
Up to $150,000
For this funding cycle, approximately 33 applicants will receive a 3 year award.
