The Colorado Health Disparities Grant Program provides funding for projects that positively affect social determinants of health to reduce the risk of future disease and exacerbating health disparities in underrepresented populations.
Funding categories include:
- Strategic planning
- Building staff and volunteer capacity
- Technical assistance for communities and grassroots organizations
- Operational support
The focus is systems or policy initiatives that align with the Healthy People 2030 initiative to address social determinants of health such as, but not limited to:
- Education
- Employment
- Social support
- Community safety
- Housing
- Transportation
- Food security
- Access to healthcare
- Environmental conditions
Eligible applicants are Colorado-based community or grassroots organizations, including:
- Nonprofits
- Healthcare organizations
- Governmental agencies
- Educational institutions
- Federally-recognized tribes
- Native American-serving organizations
Award ceiling: $190,000
Award floor: $160,000
Project period: 3 years
Estimated number of awards: 18
Estimated total funding: $3,000,000
Awards are made on a cost reimbursement basis. Recipients will be reimbursed monthly for approved expenses.
