Purpose

The Hawaii Community Foundation will provide grants for intensive, evidence-based tobacco cessation services to priority populations in Hawaii to help them to stop using tobacco.

Priority populations:

  • Native Hawaiians
  • Persons with mental health and/or substance use challenges
  • LGBT communities
  • Populations with low socioeconomic
  • Pregnant women

Project expectations:

  • Provide intensive, evidence-based tobacco treatment services to adults who use tobacco, including persons using electronic smoking devices or other emerging products containing nicotine
  • Reach and provide intensive interventions that are adapted to the needs and circumstances of the priority populations based on available best practices or evidence-based programming for tobacco cessation
  • Help people from priority populations who use tobacco become ready and willing to quit
  • Provide nicotine replacement therapies and other approved pharmacotherapy to people who use tobacco when they are ready to quit
  • Establish tobacco screening and referral partnerships with agencies in the same service area serving the same priority populations in order to increase access to services
  • Participate in evaluation of the program by working directly with the external program evaluator
  • Work with the Hawaii Tobacco Quitline to coordinate patient referrals to provide comprehensive cessation services for all people of Hawaii
  • Participate in grantee gatherings and trainings as required by HCF to increase staff knowledge, build program capacity, network with other cessation providers, and meet other grantees of the Trust Fund
  • Hire and train certified Tobacco Treatment Specialists (TTS) to perform tobacco cessation services
  • Perform all other grant expectations
Eligibility

Eligible applicants are Hawaii based 501(c)(3) nonprofit organizations with a history of successful programmatic implementation and experienced personnel.
