Rural Health Care Program – Healthcare Connect Fund
Provides assistance to healthcare providers for eligible expenses related to broadband connectivity at a flat discounted rate of 65%. Participants can apply as a member of a consortium or a stand-alone entity.

Promoting the Integration of Primary and Behavioral Health Care: States
Funding to improve the health outcomes for people with behavioral health conditions by supporting the adoption and improvement of integrated care models for behavioral and primary physical health at the state level. Tribal, rural, and other medically underserved communities are a program priority. Program eligibility is limited to states and state agencies; however, states must partner with community programs, community mental health centers, Rural Health Clinics, Federally Qualified Health Centers, and/or other primary care practices.

AHRQ Safety Program for Telemedicine
Free learning cohorts of healthcare professionals who use telemedicine as a care delivery model. The 2024 cohort will begin in June 2024 and will focus on appropriate antibiotic use while maintaining patient satisfaction and reducing potential side effects in patients seen via telemedicine. Providers and practices from rural communities are encouraged to apply.

Rural Healthcare Provider Transition Project (RHPTP)

The program's goal is to guide small rural hospitals and Rural Health Clinics that are not currently participating in VBC to prepare for participation in a health system focused on value.
