Purpose

The American Heart Association is offering grants for rural healthcare organizations in the Midwest to lead evidence-based policy and practice changes in the areas of blood pressure, cholesterol, and type 2 diabetes.

Project examples:

Eligibility

Eligible applicants are healthcare organizations in rural communities in the Midwest region.

Applicants may include, but are not limited to:

  • Health systems
  • Rural Health Clinics
  • Federally Qualified Health Centers
  • Safety net clinics

Award ceiling: $2,500
Estimated number of awards: 4

Application process

Application process:

  1. Register for at least one new/additional program within Outpace CVD™
  2. Commit to making 2024 data submission to the American Heart Association
  3. Complete the online survey to apply
